How to request a 1095-B or 1095-C form
Note: This form is not required to file your taxes.
Please note that the 1095-B / 1095-C Tax form is NOT required by the IRS for filing your federal income taxes.
As of 2026, PEBP will no longer be automatically mailing these forms to members. They are only provided upon request and may be requested using the form at the bottom of this page. By entering the requested information, you will be mailed a copy of your 1095-B or 1095-C Tax form for the last calendar year or previous year.
*The federal government requires you to have health insurance and indicate your coverage status when you file your taxes. This coverage is reported in forms 1095-B and 1095-C depending on who provided your health insurance.
*1095-B Forms are only available if you are enrolled in a PEBP medical plan. Please contact VIA Benefits if you are enrolled through the Medicare Exchange.

If you are unable to request your 1095-B or 1095-C on the form above and still wish to have a physical copy, please log into your E-PEBP Portal at https://epebp.nevadabenefits.org/ and submit a secure message under “Contact Us”.
If you are unable to log into your E-PEBP Portal, you can also request your 1095-B/1095-C by mailing your first and last name, your date of birth, a good contact phone number or email address, and your PEBP ID number (participant ID number) to:

If the information you receive on your form is not correct or has changed, you may call the Public Employees’ Benefits Program at 775-684-7000 or 702-486-3100, and dial option 2 to speak with our Member Services Unit.
